PARTICIPANT RELEASE FORM

This Participant Release Form must be filled out and signed by all participating members before entering the ambulance or participating with any of the medical equipment.

VOLUNTEER RELEASE FORM

This Volunteer Release Form must be filled out and signed by all volunteers prior to any participation in an A.K.E. event.
